Close Circuit Television (CCTV) showing wide shot of explosion at oil refinery, followed by flames and smoke STORYLINE: Newly released surveillance video shows the moment a fiery gas explosion ripped through a Utah oil refinery earlier this month. The US Chemical Safety Board on Tuesday released the video and its preliminary findings of what led to the November 4 blast at the Silver Eagle Refinery in Woods Cross. The blast damaged 10 homes but caused no serious injuries. Company officials last week heeded a recommendation from investigators to shut down the refinery until it's deemed safe to reopen. Investigators haven't made a final determination on what caused the blast. Preliminary findings indicate that the explosion happened when a 10-inch (25 centimetre) pipe failed catastrophically. The refinery in Woods Cross also had fires in 2003, 2005 and 2007, according to federal records. On January 12, four people were seriously burned at Silver Eagle when a 440-thousand gallon (1,67 million litre) storage tank caught fire and burned for 11 hours. Federal investigators said the ignition source may have been a gas heater or a refrigerator's electric outlet in a utility shed about 160 feet (49 metres) from the tank.
